For the purposes of your present and future treatment, your personal details will be documented. This may be on paper records or computer. At all times great care is taken to ensure that high standards of confidentiality are maintained. We use the information that we hold on patients for their benefit (e.g. reviewing and monitoring those with heart disease, inviting women for smear tests). We may use the information for other reasons such as research, audit, training and planning purposes. From time to time, staff that are not directly employed by the practice may have access to your records. In these circumstances the staff member will be bound by the same strict rules of confidence as the rest of the practice team.
Information held on patients may be transmitted electronically to Lothian NHS Board and to the out-of-hours service (LUCS). Any contact you make with LUCS will be logged on their computer. This information is used to manage your emergency care and monitor the LUCS service.
Please write to the practice manager if you have an objection to any of these procedures.
