Information for New Patients
To register with the practice please contact reception. If possible bring your health board medical card that shows your NHS number and previous doctor’s details.
When you join the practice you will be asked to have a “new patient check” . This is performed by the practice nurse and involves asking questions about your past and present health, checking blood pressure, height and weight, etc. It is helpful for us to know about your medical history while we are waiting for your notes to be transferred and it provides you with a useful health check.
If you are between the ages of 16 and 74 and have not been seen for three years, or are over 74 and have not been seen for one year, you are also entitled to a check-up (if the doctor or nurse feels this would be useful).
You may express a preference to consult with a particular doctor. If you wish to do so, tell the receptionist and this will be recorded on our computer system. We will try to give you an appointment with your chosen doctor but due to appointment availability this may not always possible.
