Leaving the practice

If you're leaving the practice for any reason, we're sorry to see you go!

To change practices, you simply need to submit a registration form to your new practice. You don't need to tell us that you're leaving. Your medical records will be transferred from us to them within a few weeks.

Your new practice will not receive your notes or medication list immediately. This means that it will be difficult for them to issue your repeat medications in the first couple of weeks after you register with them. To help things go smoothly, think about whether you need to need to order your repeat medications from us before you go. We'd be happy to issue a final repeat (bridging) script - just request this in the usual way that you order your repeat medications from us.
