Email your full name, date of birth, old address and new address to prescriptions.s77055@nhslothian.scot.nhs.uk Please also include the names and dates of birth of any dependants living with you that are also patients with Dalhousie Medical Practice and need to have their address changed.

Please be aware that your change of address might result in you living out with our catchment area. If you wish to check with the practice, please contact us after 2pm weekdays.
