Patient Request for Non-NHS Service

You have requested that a doctor at Dalhousie Medical Practice provides a service that is not part of core National Health Service work. Some examples of these include: statement of fitness to travel, letters for sports clubs, holiday cancellation, reports for employers or colleges. None of these above services are covered by the NHS and thus attract a fee.

• Step 1: Complete this form.
• Step 2: You will be advised of the cost and if it is a standard fee you should make payment at reception. If there is no standard fee the practice will calculate and advise you of the fee usually within two weeks. 
• Step 3: Once the fee is confirmed the GP will aim to complete within one to two weeks.
• Step 4: The practice will contact you when the request is complete.
